Your Role Includes:

  • Medical Nursing Tasks: Medication administration, wound care, catheterization,
    injections, tracheostomy and ostomy care, G-tube feeds, etc.

  • Complex Care Management: Monitoring unstable or chronic medical conditions,
    responding to clinical changes, communicating with care teams

  • Personal Care: Full scope of HCA/PSW duties including bathing, grooming,
    dressing, and mobility support

  • Client and Family Engagement: Building trust, offering emotional support,
    communicating clinical updates

  • Clinical Documentation: Using AlayaCare to track interventions, observations,
    and care notes

What You Bring:

  • A Licensed Practical Nurse Diploma from a Canadian college

  • Current registration in good standing with the College of Licensed Practical
    Nurses of Alberta (CLPNA)

  • Minimum of 3 years of clinical nursing experience, including at least 1 year in home care or community health settings in Canada, preference will be given to those with experience in Alberta; medical and/or surgical acute care experience is a strong asset

  • Expertise in caring for clients with complex medical requirements (i.e. CVADs, wound care, IV therapy, home dialysis, etc.) and client teaching.

  • Strong clinical judgment, reliability, and confidence managing complex care at
    home

  • Excellent communication skills with clients, families, and interdisciplinary teams

  • Valid Alberta Class 5 Driver’s License with access to a reliable vehicle with a clear driver’s abstract

  • Clear criminal and vulnerable sector check (within 3 months)

  • Current CPR/First Aid certification

  • Willingness to maintain up-to-date immunizations (Hep B, MMR, Tetanus,
    Influenza, COVID)

Mentorship & On-Call Support
